Terms, Conditions & Disclosure By clicking “Submit Application”, you submit and certify that all of the statements in this application are true and made for the purpose of obtaining credit. You authorize us to begin a credit investigation, including obtaining your consumer credit report, and to forward your application to lenders, financial institutions, or other third parties in order to process your application. We the Auto Dealer, may obtain information about you as described in this notice, which we handle as stated in our website privacy policy notice. If we are required to provide you our official company privacy policy, it will be given to you in person, by mail or if you consent to receive it electronically, by e-mail. This does not apply to information obtained in a non-financial transaction. All applicants must satisfy all lender criteria in order to obtain credit. Not all applicants, including pre-approved applicants, will qualify for a loan.By completing and submitting your request, you consent to having all legally required disclosures, notices, privacy policies and information (“Disclosures”) provided or made available to you in electronic form and to doing business with us electronically. We collect nonpublic personal information about you from the following sources: information we receive from you on applications or other forms in connection with a financial transaction, Information about your transactions with us, our affiliates, or others, and information we receive from a consumer reporting agency. We do not sell customer or consumer information to any third party. We may disclose the information we collect, as described above, to our affiliates, to other financial institutions with whom we have joint marketing agreements, or to companies that perform services or functions on our behalf. Delivery Authorization I hereby authorize Florida Auto Lease (“FAL”) and/or their directors, officers, employees, volunteers, representatives, vendors, and agents to pick up and deliver agreed upon vehicle to/from dealership to/from agreed upon location of delivery and assume all of the risk of having my vehicle delivered to the destination I selected. In consideration of the additional delivery fee that the company waived, in exchange for this release, and service provided, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: I waive, release, and discharge from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released and indemnify, hold harmless, and promise not to sue Florida Auto Lease Inc (“FAL”) and/or their directors, officers, employees, volunteers, representatives, vendors, and agents, for any and all claims that relate to or arise out of delivering my vehicle from its current location to the destination of my choosing. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
